Community Mental Health Nurse ADAPT Team

We have an exciting opportunity for a nurse to join the Greenwich West ADAPT Team. The successful candidate will have the opportunity to work in a busy and varied multi-disciplinary environment. You will work alongside clinical and other non-clinical colleagues to make a positive contribution to our patient's journey to recovery.

The Greenwich ADAPT Service provide care and treatment to individuals who are experience anxiety, depression, affective disorder, personality or trauma. If you are interested in developing your intervention based skills with this cohort then we would look forward to hearing from you and receiving your application.The successful candidate will be supported to develop their assessment skills and psychologically informed interventions (both group and individual) to enable them to work as an effective member of the team.

Main duties of the job

The post holder will be responsible for undertaking comprehensive assessment of service users and carers mental health and social care needs. Once these needs have been identified the post holder will be responsible for developing a goal focussed and person centred care plan to enable the provision of an appropriate intervention.

The post holder will be working in a supportive diverse team, where there are opportunities for personal and professional development.
