EXCITING OPPORTUNITY, NOT TO BE MISSED! We are looking for caring and compassionate Healthcare Assistant to become part of our Primary Care Home (PCH) team to deliver clinical interventions for patients with a wide range of health care needs in a variety of community settings.

As a Healthcare Assistant you will gain experience in many clinical skills including but not limited to wound care, venepuncture, catheter maintenance, promoting self care, administration of insulin to support our diabetic patients, palliative care, end of life care and rehabilitation support to name a few. The post holder will visit patients in their own homes or residential care to deliver these interventions either on their own or with another clinical member of staff. We will provide support and education to develop the required skills. We have a robust lone worker policy ensuring your safety whilst working within the community.

Main duties of the job

The successful applicant will work under the direction of clinicians in the Primary Care Home team to provide a high standard of nursing care. This multidisciplinary team includes Nurses, Occupational Therapists, Physiotherapists, Assistant Practitioner, Social Workers, Phlebotomists and Rehabilitation Support Workers. The Primary Care Home (PCH) is a multidisciplinary team working closely with GP's and other community partners to provide an excellent standard of person centred care to adults in their own homes, residential care or clinic settings, using a health coaching approach to support patients to self manage whenever possible. The service uses SystmOne as a clinical IT system to support mobile working.

To implement the care as planned taking into account own competencies.

To deliver safe patient centred care within a variety of settings.

Take, record and report blood pressure, respiration, pulse rate, oxygen saturation levels. Be able to recognise the deteriorating condition of a patient.

Undertake and record MUST and Waterlow assessments and take appropriate action.

Be able to participate in wound care, skin tear pathway, venepuncture, collection of specimens or diabetic foot assessment as required.

To report any concerns / changes in the patients condition to a registered nurse or appropriate Health care Professional without delay.

Ensure a high standard of record keeping is achieved in line with ECCH standards.

Engage in personal development, maintain competence, knowledge and skills commensurate with role.

Take responsibility for the safe use of clinical equipment.

Contribute to the effective use of resources.
