Job summary

A leading healthcare provider in St. Austell is seeking a Homefirst Assessor for a full-time/part-time position. This role focuses on the assessment, planning, and implementation of care within the community. Ideal candidates must be HCPC registered and have substantial clinical experience. Responsibilities include working with multidisciplinary teams to provide evidence-based care to patients in their homes or care facilities. Flexibility with working days and coverage from 8am-6pm is required. Salary ranges from £38,682 to £46,580 annually.

• Experience of managing risks in lone working environments.

Responsibilities

• Conduct assessments and implement care plans for patients.
