We are seeking a reliable anddetail-oriented Medical Coder to support the clinical team by ensuring accurateprocessing, coding and management of patient information within EMIS. The postholder will be responsible for reviewing clinical correspondence, assigning thecorrect SNOMED codes, updating patient records and following establishedpathways to ensure information is recorded safely and efficiently.
The role involves close collaborationwith clinicians, the scanning team and administrative colleagues to maintainhigh standards of data quality, support QOF and clinical audits and ensuretimely coding completion. The ideal candidate will have excellentattention to detail, the ability to work independently, strong organisationalskills and a commitment to accuracy and confidentiality. Prior experiencein medical coding or familiarity with EMIS and primary care terminology isdesirable but not essential, as training will be provided.
Main duties of the job
- Review incoming clinical correspondence (hospital letters, discharge summaries, clinic reports, etc.) and extract key clinical information.
- Summarise transferred paper records (e.g., Lloyd George notes) from previous practices, identifying significant medical history, medications, allergies, surgical history, safeguarding concerns, and other relevant details.
- Assign accurate SNOMED CT codes for diagnoses, procedures, tests, referrals, and relevant medical history.
- Follow practice-specific pathways to route documents to clinicians or other teams as required.
- Check for urgent or abnormal findings and expedite them through the appropriate pathway.
- Manage tasks and workflow queues to ensure timely completion of coding actions.
- Support QOF, enhanced services, and clinical audit requirements by coding or flagging relevant information.
- Identify missing information or discrepancies in patient notes; take corrective action or seek clarification.
- Maintain confidentiality, GDPR compliance, and adherence to practice policies at all times.
- Work closely with the scanning team and wider administrative team to maintain consistent data quality.
- Participate in ongoing training, updates, and quality assurance activities related to coding and data accuracy.
- Enter and maintain complete, accurate patient records in EMIS, ensuring all data is correctly filed and ready for safe clinical use.

Job responsibilities
Summarising
- Maintain patient medical records on EMIS in chronological order, in good repair, and archive older records safely.
- Summarise clinical records and update the electronic patient record accurately and promptly, following protocol.
- Identify and elevate any anomalies in patient notes.
- Ensure unnecessary information is disposed of appropriately.
- Sort records and letters in preparation for summarising.
- Respond to staff queries and ensure timely completion of record summaries.
Clinical Coding
- Accurately code all written and electronic clinical correspondence.
- Direct scanned documents to the correct clinician or team using established pathways.
- Stay up to date with pathway changes and proactively follow procedures.
- Liaise with the line manager on any issues with scanned or electronic records.
Admin Duties
- Complete workflow tasks accurately and efficiently.
- Record the arrival of new records delivered by PCSE.
- Undertake any other appropriately delegated task.
- Work closely with clinicians and admin staff to ensure high-quality patient records.
Practice Development and Clinical Governance
- Participate in primary healthcare team meetings.
- Provide data as requested for Practice Development Plans and reports.
- Support clinical and organisational audit process.
- Follow policies on risk management, incident reporting and health and safety.
- Be aware of own development needs and participate in training and education opportunities.
- Operate the practice complaints procedure and respond or forward complaints as appropriate.
