A prominent healthcare organization in Daventry is looking for a Clinical Pharmacist to join their GP team. This part-time position will involve improving patient health outcomes through medication management and supporting the clinical staff. The ideal candidate should have significant pharmacy experience and hold relevant qualifications. The role promises diverse responsibilities within a supportive environment.
We are looking to recruit a Clinical Pharmacist to support our GP team at Abbey House Medical Practice.
The post holder is a Clinical Pharmacist who acts within their professional boundaries, working alongside a team of pharmacists in general practice within the wider PCN.
They will improve patients health outcomes and the efficiency of the primary care team by providing direct, accessible and timely medicines and expertise including face to face contact with patients.
The post holder will be an integral part of the general practice team, as well as part of a wider pharmacist network and multi-disciplinary team. They will work to optimise medication issues to improve patient care and safety and support clinical staff in the management of patients.
As part of their employment, the post holder will achieve qualifications from an accredited training pathway, equipping the Clinical Pharmacist to be able to practise safely and effectively in a primary care setting.
This is a temporary position - Monday to Friday, part-time hours working 3 days a week.
Salary depending on experience.
Clinical pharmacists have a key role in supporting delivery of the new Network Contract DES Service specifications. For the new Structured Medications Review and Optimisation requirements this will include tackling over-medication of patients, including inappropriate use of antibiotics, withdrawing medicines no longer needed through NHS-led programmes such as low priority prescribing and medicines optimisation more widely. For Enhanced Health in Care Homes residents will benefit from regular clinical-pharmacy led medicines reviews.

KEY RESPONSIBILITIES
Towork as part of a multi-disciplinary team in a patient-facing role toclinically assess and treat patients using expert knowledge of medicines forspecific disease areas.
Takeresponsibility for the care management of patients with chronic diseases andundertake clinical medication reviews to proactively manage people withcomplex polypharmacy, especially the elderly, people in care homes, those withmultiple long term conditions and people with learning disabilities or autism(through STOMP - Stop Over Medication Programme).
Providespecialist expertise in the use of medicines while helping to address boththe public health and social care needs of patients and help in tacklinginequalities.
Provideleadership on person centred medicines optimisation (including ensuringprescribers in the practice conserve antibiotics in line with local antimicrobialstewardship guidance), medication monitoring and quality improvement, whilecontributing to the quality and outcomes framework and enhanced services.
Throughstructured medication reviews support patients to take their medications toget the best from them, reduce waste and promote self-care.
Havea leadership role in supporting further integration of general practice withthe wider healthcare teams (including community and hospital pharmacy) tohelp improve patient outcomes, ensure better access to healthcare and helpmanage general practice workload.
ADDITIONALRESPONSIBILITIES
Developrelationships and work closely with other pharmacists across networks and thewider health system.
Takea central role in the clinical aspects of shared care protocols, clinicalresearch with medicines, liaison with specialist pharmacists (including mentalhealth and reduction of inappropriate antipsychotic use in people with learningdifficulties) and anticoagulation, as well as medication monitoring processes.
Communicate effectively with patients and carers,recognising the need for alternative methods of communication to overcomedifferent levels of understanding, cultural background and preferred ways ofcommunicating.
Adhere to quality governance systems, processesacross the Practice and its activities; managing data collection to supportdata requirements/claims; ensuring information governance standards areimplemented and maintained.
Ensure health and safety requirements are met.
Attend training courses identified as essential orbeneficial to the role and take responsibility for identifying own learningneeds.
Ensure all significant events are reported anddiscussed as appropriate and facilitate a full understanding of clinicalgovernance requirements within the practice.
ADDITIONAL INFORMATION
Thepost holder holds a position of importance, being the crucial link between thedoctors, staff and patients.
Youwill be expected to attend regular staff meetings. The meetings provide anopportunity to discuss matters arising from the job and can be used forin-house training.
Thepost holder will be expected to deliver a high quality, diverse, patientcentred service.
