Job description

Clinical pharmacists will have a key role in supporting delivery of the new Network Contract DES Service specifications. For the new Structured Medications Review and Optimisation requirements this will include tackling over‑medication of patients, including inappropriate use of antibiotics, withdrawing medicines no longer needed through NHS‑led programmes such as low‑priority prescribing and medicines optimisation more widely. For Enhanced Health in Care Homes residents will benefit from regular clinical pharmacy‑led medicines reviews.

Remote working does not form part of the role and the successful candidate would be expected to work from the PCN practices, which are in the Romford area.

Main duties of the job

To deliver a high standard of patient care as a Clinical Pharmacist in general practice, using advanced autonomous clinical skills, from a broad in‑depth theoretical knowledge base who works collaboratively to meet the needs of patients, supporting the delivery of policy and procedures whilst managing their own workload to deliver the practice priorities and vision.

Job responsibilities

KEY RESPONSES

  • To undertake clinical medication reviews for patients with chronic diseases and to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple long‑term conditions.
  • Provide expertise in the use of medicines and person‑centred medicines optimisation across the network, including taking a central role in the clinical aspects of shared care protocols and liaison with specialist pharmacists to reduce inappropriate use of high‑risk medicines.
  • Support the integration of general practice with wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, reduce waste, and promote self‑care, provide better access to healthcare, and manage general practice workload.
  • Network wide:
    • Managing Safety and CAS alerts
    • Assisting with QOF and incentive scheme audits, PINCER safety indicators
    • Managing availability queries
    • Attending Medicines Management/Prescribing forums quarterly and assisting practices in achieving Prescribing Incentive Scheme targets.
    • Attending PCN meetings as needed.
  • Practice specific:
    • CQC preparedness e.g. ensuring all prescribing protocols are up to date
    • Care home support e.g. annual meds review of patient
    • Discharge summary reconciliation
    • Polypharmacy Medication reviews
    • Supervision and training of reception staff in issuing of repeat prescriptions and queries
    • Supervision and training for new prescribing staff including trainees and nurse prescribers
    • Attending Clinical and PCN meetings as required
    • Telephone advice to patients for queries on medication and minor illness, self‑care, sign‑posting to other services as needed.
    • Review of repeat prescription policies and support staff in implementation.
Person Specification

Qualifications

  • MPharm, Independent Prescriber desirable but not essential and current registration with GPhC
Experience
  • Minimum of 2 years; in‑depth therapeutic and clinical knowledge and understanding of evidence‑based healthcare.
  • An appreciation of the nature of primary care prescribing and concepts of medicines optimisation.
