Job summary

A leading healthcare provider in the UK is looking for a Clinical Pharmacist to offer high-quality services in admissions. Responsibilities include medication reconciliation, promoting medication reviews, and ensuring patient safety. Candidates should have relevant hospital pharmacy experience and preferably a postgraduate diploma in General Pharmacy Practice. This role supports teamwork and requires excellent communication. Working hours include rotations on weekends and bank holidays.

  • Completion or working toward a Post Graduate Diploma in General Pharmacy Practice or an equivalent qualification.
  • Enjoy working as part of a multidisciplinary team.

Key Responsibilities
  • Provide a high quality Clinical Pharmacy service to the modern Admissions Units, reconciling medicines prescribed on admission, promoting early in‑depth medication review and supporting the discharge process to facilitate high quality patient outcomes, reduce prescribing errors and ensure patient safety.
Required Experience and Qualifications
  • Post‑registration experience relevant to hospital Pharmacy.
  • Completion or working toward a Post Graduate Diploma in General Pharmacy Practice or an equivalent qualification.
  • Excellent communication, interpersonal and time‑management skills.
  • Enjoy working as part of a multidisciplinary team.
  • All posts working with vulnerable groups will be subject to an enhanced criminal records check upon appointment; staff must consent to the Trust receiving regular updates on their criminal record status throughout employment.
Working Hours and Schedules

Monday to Friday, participation in weekend (Saturday and Sunday) and Bank Holiday rotations as required.
