A healthcare organization in Droitwich is seeking an experienced Clinical Pharmacist to join their multidisciplinary frailty team. This permanent, part-time role focuses on delivering expert medicines optimisation for older adults living with frailty. Droitwich, Ombersley & the Rurals (DO&R) Primary Care Network has an exciting opportunity for an experienced Clinical Pharmacist to join our established multidisciplinary frailty team. This permanent, part‑time role (22.5 hours per week) focuses on delivering expert medicines optimisation and clinical pharmacy support for older adults living with frailty across care homes and the wider PCN.
Working as a core member of the PCN frailty MDT, you will provide structured medication reviews, polypharmacy management and deprescribing support, working closely with GPs, Advanced Practitioners, nurses, social prescribers, community services and care home staff. The role supports proactive, personalised care to improve patient outcomes, reduce medicines‑related harm and prevent avoidable hospital admissions.
You will contribute to current PCN contracts and frailty pathways, participate in MDT meetings and case conferences, and support service development, quality improvement and prescribing safety initiatives. This role offers the opportunity to apply advanced clinical pharmacy skills within a supportive PCN environment, with access to supervision, appraisal, continued professional development and the NHS Pension Scheme.
Salary range £46,226 to £53,846 dependent on qualifications and experience.
