Overview

Job summary

The post holder will provide primary support to general practice staff with regard to prescription and medicines management at different sites. They will help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation on the transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practice. They will deliver an efficient, cost-effective, and high-quality clinical service by providing clinical and professional development to the clinical workforce within K&W Healthcare & member practices. The role is pivotal to improving the quality of care and operational efficiencies.

The candidate should be motivated and passionate about delivering excellent service within general practice.

Key Responsibilities
  • Clinical reviews/input: Demonstrate a significant reduction in GP time spent by seeing patients in appointment slots for clinical medication reviews and urgent medication requests. Complex patients or those with additional needs may require longer appointment slots.
  • Review the ongoing need for each medicine, monitoring needs, and support patients in getting the best use of their medicines.
  • Minor ailments triage: Manage caseload of patients with common/minor/self-limiting ailments requesting GP appointments and triage appropriately.
  • Cardiology interest: Stay up-to-date with cardiovascular treatments, guidelines, and best practices relevant to patient care.
  • Provide patient-facing clinics for questions or concerns about medications to improve adherence.
  • Minor ailments appointment slots as appropriate.
  • Contribute to multidisciplinary reviews about medication-related issues.
  • Reconcile medicines following hospital discharge and review daily pathology results for patients on known medicines.
  • Targeted medication reviews for high-risk patients (frail elderly, polypharmacy, renal and hepatic impairment, high-risk medicines, etc.).
  • Undertake clinical medication reviews with patients and produce recommendations for nurses and GPs on prescribing.
Essential Qualifications
  • 1st degree – Vocational master's degree in pharmacy (4 years)
  • Registered with the General Pharmaceutical Council (GPhC)
  • Previous pre-registration training
  • Completed or working towards completing the CPPE pathway
  • Evidence of continuing professional development to meet mandatory GPhC requirements
  • IT skills at least ECDL standard or equivalent
  • CPPE Pathway: mandatory enrollment; provide completion certificate if completed, or an official exemption certificate if not undertaking it
Desirable
  • Member of Royal Pharmaceutical Society or other professional groups
  • Independent prescribing qualification or commitment to achieving this
  • Significant post-registration experience
  • Experience in primary care and/or hospital and community pharmacy
Skills

Experience
  • Significant post-registration experience
  • Knowledge and experience of pharmacy within primary care and/or hospital and community settings
  • Familiarity with NHS organisation operations
Public Health / Medicines Optimisation Related Areas
  • Support public health campaigns and provide specialist knowledge on public health programs
  • Participate in medicines optimisation systems and audits; liaise with GPs and non-medical prescribers
  • Provide medicines information to practice teams and patients; support formulary alignment and CQC compliance
  • Reconcile medicines post-discharge and ensure continuity of supply for high-risk groups
