Overview

We are seeking dedicated and knowledgeable Clinical Pharmacists to join our healthcare team. The ideal candidates will play a crucial role in ensuring the safe and effective use of medications for patients. This position requires a strong foundation in anatomy, medication administration, and patient care to provide optimal pharmaceutical services. These roles are full/part time and a mix of remote and practice based working.

Responsibilities
  • Completing medication reconciliations and reviews for patients SMR both in person and over the telephone
  • Medicine optimisations and medicine RX
  • Prescribing medication
  • Must be able to offer sound and professional advice in relation to clinical management issues
  • Maintain and improve quality and patient safety
  • Provide pharmaceutical input for clinical governance meetings

Qualifications
  • A degree in Pharmacy from an accredited institution.
  • Registration with the General Pharmaceutical Council (GPhC) or equivalent body.
  • Strong knowledge of anatomy and physiology relevant to medication management.
  • Experience in medication administration within a clinical setting, preferably in a primary care environment.
  • Excellent communication skills with the ability to work collaboratively within a multidisciplinary team.
