Clinical Pharmacist – Primary Care Network (Nuneaton)
We are seeking a motivated and experienced Clinical Pharmacist to join our dynamic primary care team in Nuneaton. This is a practice-based role within a collaborative network, offering the opportunity to make a real impact on patient care.
Overview
Clinical Pharmacist role within a primary care network, focusing on medicines optimisation, patient safety, and collaborative care across the practice and community teams.
Responsibilities
- Conduct clinical medicines reviews, focusing on high-risk patients such as those with polypharmacy, frailty, renal or hepatic impairment, and complex medication regimens.
- Reconcile patient medications following hospital discharge and liaise with community pharmacy colleagues to ensure continuity of care.
- Provide expert advice to prescribers, patients, and healthcare teams on medication use, safety, and alternatives.
- Lead initiatives to improve prescribing practices, implement clinical guidelines, and support quality improvement through audits.
- Manage repeat prescriptions, electronic repeat dispensing, and re-authorisation within scope of practice.
- Contribute to clinics for long-term condition reviews, multi-morbidity assessments, and patient education.
- Act as a medicines information resource for the practice team and patients.
Requirements
- Registered Pharmacist with GPhC registration and membership of the Royal Pharmaceutical Society.
- Minimum 2 years post-graduate experience in pharmacy, including at least 1 year in primary care or general practice.
- Experience in clinical assessment, medicines optimisation, and working in multidisciplinary teams.
- Strong communication, influencing, and relationship-building skills.
- Desirable: independent prescribing qualification, postgraduate pharmacy qualification, EMIS experience, and supervisory experience.
