A well-established PCN in Nottingham is seeking a pharmacist to join their supportive clinical pharmacy team. You will manage chronic disease treatment and undertake clinical medication reviews. Job description

We are a well-established, forward thinking PCN with a large and supportive clinical pharmacy team, with the depth to provide support for newly qualified pharmacists or the opportunity for growth and development for more experienced pharmacists. Medicines Safety is at the heart of our team, working at PCN level to ensure high risk medicines are monitored regularly, along side innovative system one visualisations on drug monitoring. You will be a pharmacist who will act within their professional boundaries supporting and working alongside a team of Pharmacists in General Practice and supported by a Neighbourhood Lead. Initially you will be based within two practices, however PCN staff can be given the opportunity to cover different practices or the pharmacy cardiology team depending on the need of the service. Working as part of a multi‑disciplinary team in a patient facing role, you will be responsible for chronic disease management, undertaking clinical medication reviews to proactively manage patients with complex polypharmacy. You will provide primary support to general practice staff with regards to prescription and medication queries by supporting the repeat prescriptions system, dealing with acute prescription requests, medicine reconciliation and support the QOF and IIF targets. The post holder will be supported in enrolling on and completing the CPPE Primary Care Pharmacy Education Pathway and then an independent prescribing course, unless already completed.

Responsibilities

  • Support the repeat prescriptions system, address acute prescription requests, perform medicine reconciliation

  • Provide clinical leadership on medicines optimisation and quality improvement
Qualifications
  • Master's degree in Pharmacy (MPharm)
  • Mandatory professional registration with the General Pharmaceutical Council
  • Knowledge and understanding of the nature of GP Practices
  • Enrolment and completion of the CPPE Primary Care Pharmacy Education Pathway and Independent Prescribing course (if not already completed)
