Job description

The post holder will be responsive and flexible, working collaboratively with other members of the hospice multi‑disciplinary teams, to provide person‑centred care focussing on what matters most to patients and those important to them. You will

Responsibilities
  • Provide specialist complex palliative nursing care in the last months, weeks and day of life to patients in the community.
  • Work autonomously in the community providing specialist assessments of patients with complex needs.
  • Deliver high‑quality nursing care in patients’ homes and act as a team leader in the management of a complex caseload as directed by the Senior Clinical Nurse Specialists.
  • Perform comprehensive assessments and prescribe appropriately if a non‑medical prescriber.
  • Lead on and support junior staff within the management of complex patients, supporting them to prioritise needs.
  • To assist the Integrated Community Service Leads in evaluating and developing specialist palliative care services further.
  • Work autonomously during weekends and as part of a 7‑day rota, including bank holidays.
  • Collaborate closely with the wider multi‑disciplinary team (MDT).
  • Attend GP Gold Standard Framework meetings.
  • Attend and chair capacity and multi‑disciplinary meetings.
Qualifications
  • Experienced in palliative care and ideally has community experience.
  • Resilient and confident working independently and autonomously, including working at weekends and bank holidays.
  • Committed to developing professionally within a supportive and value‑led environment.
  • Able to manage work time effectively and lead on and manage efficient day‑to‑day coordination of junior staff.
  • A driving licence holder and has access to a vehicle.
