Job description

We are seeking enthusiastic and reliable individuals to join our team as part-time Chronic Disease Nurse. You will be working as a team of practice nurses. This role primarily involves managing patients on chronic disease registers.The ideal candidate / extended skills in Chronic Disease Management in Diabetes, COPD, Asthma, CHD.

Main duties of the job

All aspects of Chronic Disease Management , including the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for patients.

The following are the core responsibilities of Chronic Disease Management / Practice Nurse. There may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels.

Person Specification

Experience

• Experience of working in a healthcare setting.
• Experience of dealing with vulnerable patients.
• Experience of working as a Practice Nurse, in a primary care environment. Experience of Chronic Disease Management- Asthma, COPD, Diabetes, CHD.

Qualifications

• Good standard of general education Registered Nurse with the NMC
• Diabetes, Asthma / COPD
