Qualifications

  • 2 years of paid experience working with people with disabilities or chronic illnesses.
  • 5 years of experience with a bachelor's degree in an unrelated field.
  • Valid NYS Driver’s license required.

Responsibilities

Salary: $20.00-22.00/hour depending on years of experience/education level.

About the Role

Provides care management services to children living in Livingston, Steuben, and Wyoming counties who have been diagnosed with one or more chronic illnesses, serious emotional disturbance, OR HIV/AIDS and who meet appropriateness criteria.

Key Responsibilities
  • Provide direct care management services, maintaining accurate and verifiable documentation of all services rendered.
  • Conduct visits with clients in their chosen setting: home, school, or community approximately three days per week.
  • Accept referrals as assigned by Program Manager in Livingston, Steuben, and/or Wyoming Counties.
  • Use assigned office as home base: Mt. Morris, Hornell, or Bath location.
  • Complete comprehensive and detailed assessments, reviews, crisis plans, and care plans as required by standards set by DOH and lead Health Home contractors.
  • Will conduct additional assessments including, but not limited to, Child and Adolescent Needs and Strengths – New York (CANS-NY), Safety/Crisis, and Comprehensive Assessments.
  • Oversee implementation of plans to the satisfaction of the clients served.
  • Provide one to two core services for each client monthly as required or when needed, to ensure all care plan goals are addressed.
  • Maintain telephone availability for clients during the workday.
  • Assist clients with applications and paperwork for and management of benefits, financial, housing, or other related areas as needed.
  • Arrange for and manage coordination of medical care and other services as needed for clients.
  • Assist clients in accessing community-based support.
  • Communicate with management staff regarding intakes and placement of clients within each program.
  • Ensure adequate coordination, appropriate communication and maximum cooperation between all sources of support and services.
  • Provide on-call availability via cell phone to Health Home program as needed. Serve as client advocate.
Qualifications
  • Bachelor’s degree in human services or related field and two years of paid experience working with people with disabilities and/or chronic illnesses.
  • Bachelor’s degree in an unrelated field with five years of paid experience working with people with disabilities and/or chronic illnesses.
  • Associate’s degree in human services or related field and two years of paid experience working with people with disabilities and/or chronic illnesses.

A relevant combination of education and experience will be considered.

Skills
  • Demonstrate ethical business practices, in conformance with all state and federal laws and regulations.
  • Possess excellent verbal and written communication skills.
  • Previous experience working with diverse populations, including low to moderate-income families.
  • Ability to travel in all weather conditions throughout the counties served.
  • Willingness to foster agency, department and program wide cooperation and teamwork using positive/constructive communication techniques.
  • Ability to maintain absolute confidentiality of all records reviewed including consumer records, employee records and billing records.
  • Ability to analyze and interpret data and to handle problem resolution.
  • Possession a valid NYS Driver’s license and a driver\'s record considered acceptable by agency and insurance carrier.
