Nurse Case Manager – Complex Community Care (South London/South West London)

Salary: £45,000 per year. Excellent benefits.

We are seeking an experienced clinician looking to step into a rewarding leadership role within complex community care. You will play a vital role in shaping care packages that truly change lives across South and South West London.

Responsibilities
  • Lead and oversee a caseload of adult clients requiring complex community care.
  • Develop personalised care plans, provide clinical oversight, and support and clinically upskill care teams.
  • Collaborate closely with CHC teams, commissioners, multidisciplinary professionals, and families to deliver holistic, outcome‑focused care.
  • Support clients with complex needs (e.g., ventilation, tracheostomy care, PEG and NG feeds, spinal injuries, neurological conditions).
  • Monitor, assess and review clients’ physical and emotional wellbeing, ensuring safe medication management and clinical interventions.
  • Build strong, professional relationships with families, CHC nurses, and commissioners.
  • Contribute to service improvement and uphold high compliance standards.
Qualifications
  • Registered Nurse (RGN) with an active NMC PIN.
  • Clinical background in complex community care or acute complex care.
  • Recent experience in tracheostomy and ventilator care.
  • Empathetic leadership style with experience supporting HCAs or support workers.
  • Excellent communication and clinical assessment skills.
  • Suitable for Band 5 or Band 6 nurses ready to progress into a leadership role.
  • Full UK driving licence.

  • Hybrid working: 1 day WFH, 3 days in the community, 1 day at head office (Wimbledon).
