A busy GP practice in Crewe is offering an exciting opportunity for a Care Navigator to join their team. The role involves meeting and greeting patients, arranging appointments, and providing general administrative support. Meeting and Greeting patients, arranging appointments and dealing with general administration for the practice.
We are a busy GP practice within the Crewe area. We offer a variety of patients services to our population.
Care Navigators are responsible for undertaking a wide range of reception dutiesand the provision of general support to the multidisciplinary team. Duties can include but are not limited to,greeting and directing patients, effective use of the appointment system,booking appointments, processing of information and assisting patients asrequired. To act as the central point ofcontact for patients, the distribution of information, messages and enquiriesfor the clinical team, liaising with multidisciplinary team members andexternal agencies such as secondary care and community service providers.
