Job description
Overview

We are looking to recruit a Care Manager to lead and develop complex care services within a community setting. As a Care Manager, you will oversee a caseload of clients with complex health needs, ensuring that every care package is safe, person-centred, and responsive to changing requirements. The Care Manager will take ownership of transitions, start-ups, and the ongoing management of care packages, working closely with families and multi-disciplinary teams to deliver the highest standards of support. You will regularly review care plans and risk assessments, maintaining full compliance with safeguarding and regulatory standards. In this role, you will also provide leadership, mentoring, and development opportunities for care teams, ensuring staff are trained and confident to deliver exceptional care in the community.

Responsibilities
  • Manage a caseload of clients with complex health needs in a community setting.

Qualifications
  • Experience in managing complex care packages, including conditions such as spinal injuries, acquired brain injuries, motor neuron disease, or multiple sclerosis.
  • Strong knowledge of CQC regulations, compliance, and safeguarding practices.
  • Proven leadership skills with the ability to motivate and mentor care teams.
  • Excellent organisational and time management abilities.
  • A people-first approach with passion for delivering exceptional care.
  • A full driving licence and access to your own car.
