Job title: Team Leader
Location: Hazlemere, High Wycombe
Hours: Full-time – 39 hours per week
Pay: £14.09 per hour
We’d love to hear from you! We have an exciting opportunity for a Team Leader to join our team on a full-time basis, based at our service in High Wycombe. The main purpose of the role is to provide leadership and management to the care staff team, ensuring our service users receive the highest quality care and support in line with their individually assessed needs.
Responsibilities
- Lead on and develop person-centred Support Plans based on assessed needs and mindful to the wishes of the service user.
- Ensure the process of regular reviews of Support Plans with input from relatives, other professionals and staff colleagues is completed.
- Update service user information in daily reports, care and health monitoring charts, accident and incident reports, and service message systems promptly and clearly at all times.
- Take a practical approach in assisting service users with daily living tasks such as personal care, mobility, nutrition and hydration and end of life care with dignity and respect as directed in the Support Plan.
- Ensure service users receive planned or urgent health care whenever required, liaising with other support services e.g. District Nurses, where necessary.
- Administer medication as prescribed to service users and assist in managing an effective system for the safe control and administration of medication in line with policy and procedures.
- Support service users socially and emotionally and ensure service users are included in activities of their choice and in accordance with their Support Plan.
- Effectively lead, motivate and support care staff by facilitating “key sessions” with the guidance of the Line Manager.
- Contribute to the smooth running of the Care Home by undertaking daily tasks and routines and participating in team meetings or team initiatives.
- As a representative of the Trust, communicate with everyone living, working or visiting the Care Home clearly, helpfully and respectfully.
- Undertake any such duties or tasks consistent with the operational business of the Trust, the job role and hours of work as may be reasonably required.

Qualifications and Experience
- A minimum of 2 years’ experience working within a health or care service setting providing high quality care.
- Ability to write and deliver effective care and person-centred care support plans.
