Job description

The Care Coordinator - Safeguarding post provides arewarding opportunity to work alongside GPs and Practice teams to developefficient processes and systems at the very heart of general practice.

Provide central co-ordination for safeguardinginformation and communication to help GPs plan care for vulnerable patients.

Maintain consistent protocols for safeguardingadministration, document handling, record management and communication withpartner organisations.

Support multi-disciplinary team working -communicating between teams who have a joint role in looking after vulnerablepatients; the Primary Care Network team, Multi Agency Safeguarding Hub and theCCG.

Main duties of the job
  • Safeguarding policies -Help the practice to ensure safeguarding policies and procedures are upto date and consistent with agreed best practice
  • Safeguarding Training -Help the practice run training events whichencourage a multidisciplinary approach to safeguarding.
  • Information Governance -Provide advice and training on the recording and coding of safeguardingrelevant information within the electronic patient record.
  • Safeguarding Assurance -Support the practice in achieving CQC compliance with respect to thesafeguarding of staff and patients.
  • MDT facilitator -Enable and support the development and running of an effective practicesafeguarding MDT for children and vulnerable adults.
  • Working together -Help the practice foster effective relationships with local safeguardingpartners. Ensure the timely sharing of relevant information, both verbally andin a structured report, when safeguarding concerns arise in respect of bothchildren and adults.
  • System Learning -Support the practice in responding to statutory requests for informationrequired by the local authority to carry out local child or adult safeguardingcase reviews and assist the practice with section 42 enquiries. Disseminatelearning and support the implementation of review recommendations.
  • Referral Pathways -Promote the understanding of local safeguarding referral pathways,including Early Help and support services for victims of domesticabuse.

Job responsibilities

To oversee the workflow of safeguardingreports and documents and ensure they are circulated to appropriate cliniciansin line with practice safeguarding policies.

To triage incoming information fromAdult Services in regards to vulnerable adults, including completing MARACreports, hospital DNA/WBN letters, police SCARFs and any other information relevantto the safeguarding department.

To summariseand code the documents into the relevant specific templates, highlighting topatients registered GPs anything significant.

To ensure thecorrect coding is added to patient records to aid clinicians in identifyingthose children and adults who are cause for concern.

To add alertsto parent records, those who have a child on a safeguarding plan.

Any othertasks delegated by practices, suitable to the candidates strengths.

To contributeto data collection and audit in relation to safeguarding to support and improvestandards for safeguarding practice across the PCN.

Plan andprovide administrative support for the weekly safeguarding huddle, MDT to be chairedby the Senior GP Partner, including minute taking and agenda items.

Invitationsto Initial / Review Child Protection Conference.

Requests forreports for Initial / Review Child Protection Conferences.

General

Responsiblefor reviewing and processing all incoming proxy access requests, which allow a nominatedindividual (such as a parent, carer, or legal representative) to access apatients medical records via online services. This includes verifying thelegitimacy of the request, ensuring appropriate consent and documentation arein place, and assessing any safeguarding risks in line with practice protocolsand national guidelines.

Proxy accessrequests for care homes.

Working withthe Duty Manager to ensure patient consent is correctly recorded on SystmOneOnline.

Activelyparticipates and takes minutes in Gold Standard Framework (GSF) meetings tosupport the identification, care planning, and coordination of patients nearingthe end of life, ensuring a multidisciplinary approach to high-qualitypalliative care.
