Position

Are you a compassionate Registered Nurse with a passion for providing exceptional care coordination services? We are seeking a dedicated Care Coordinator to join our dynamic team. In this role, you will have the opportunity to directly impact the lives of our Federal Long-Term Care Insurance Plan (FLTCIP) enrollees by assessing their needs, developing personalized care plans, and ensuring they receive the maximum benefits from their insurance coverage.

Our Care Coordination Department plays a vital role in assessing and approving long term care services, developing personalized care plans, and ensuring our enrollees receive the maximum benefits from their insurance coverage. We advocate for our clients, educate them on plan benefits, and provide responsive support to meet their healthcare needs.

Your impact

As a Care Coordinator, you will interact directly with insured individuals, their representatives, and healthcare providers. You will be responsible for evaluating benefit eligibility, coordinating care, educating on insurance processes, and offering necessary referrals. Your role will involve making informed decisions based on policy guidelines, documenting exceptions when necessary, and collaborating closely with our team to ensure timely and accurate resolutions.

Key Responsibilities
  • Clinical Services (50% of time): Gather and assess information to determine benefit eligibility. Develop personalized care plans based on individual needs and document any policy exceptions. Provide timely updates to Team Lead on critical issues.
  • Communication (40% of time): Explain insurance plans and benefits, assist enrollees in maximizing their insurance coverage, and respond to inquiries via phone and written communication. Act as an advocate for enrollees to ensure their needs are met effectively.
  • Clinical Risk Management (10% of time): Identify and escalate potential legal, fraud, or customer complaint issues. Participate in high-risk reviews to uphold compliance and integrity in all interactions.
Qualifications
  • Registered Nurse with 5+ years of experience in various medical settings, preferably in long term care or home health.
  • Experience in Case Management is preferred.
  • Strong assessment skills and professional judgment.
  • Excellent verbal and written communication skills, with previous telephonic experience advantageous.
  • Proficient in MS Office Suite and basic computer skills.
  • Completion of AHIP Long Term Care courses within 3 years of employment.
  • Remote work experience is a plus
Work location and schedule

Location: Portsmouth, NH - HYBRID 2 days in office

Full-Time Position M-F with core business hours
