This is a small, friendly GP Practice. No two days are the same. We want you to come and join our team!

Main duties of the job

We are seeking a full-time fixed-term for 12 months Care Coordinator to join our amazing team.

Job share will be considered.

Therole will primarily focus on triaging GP online consultation requests,supporting the clinical team through prescribing clerk duties, and assistingwith wider administrative and reception responsibilities when required.

This is apatient-facing non-clinical role that requires excellent organisational skills,attention to detail, and a good understanding of General Practice processes.Experience of working in a GP surgery is essential, and experience withclinical triage systems is desirable.

This role would be suited to someone who has worked in a medical field.

Working Pattern

  • 37.5 hours per week (job share 2 roles available).
  • Core hours between 8:00am 6:30pm, flexible shifts to be agreed.
  • Competitive hourly rate: £13 per hour.

Job responsibilities

Online Consultation Triage

  • Review, triage, and allocate online consultation requests to the appropriate clinician or service in a timely manner.
  • Identify urgent or red flag symptoms using practice protocols and escalate to the duty GP as appropriate.
  • Communicate effectively with patients via phone, SMS, or email regarding their requests.
  • Process repeat prescription requests accurately and safely in line with practice protocols.
  • Liaise with patients, pharmacies, and clinicians regarding prescription queries.
  • Support medication reviews and prescribing audits under GP guidance.
  • Ensure prescription records are accurate and up to date on EMIS.

Administration and Reception

  • Provide cover for reception and admin colleagues during busy periods, annual leave, or sickness absence.
  • Register new patients and process changes to patient records.
  • Deal with incoming calls, emails, and correspondence in a professional manner.
  • Support clinical and management staff with general admin tasks, scanning, coding, and document management.

Care Coordination

  • Act as a point of contact for patients requiring additional support navigating health and social care services.
  • Signpost patients to appropriate services and ensure care plans are updated.
  • Work closely with the practice team to improve patient access and continuity of care.

General

  • Work flexibly to meet the needs of the practice and wider team.
  • Maintain confidentiality and comply with GDPR and NHS information governance standards.
  • Participate in practice meetings, audits, and service improvement initiatives.
  • Undertake mandatory training and continued professional development as required.
Person Specification
Experience
  • Experience working in General Practice (clinical or non-clinical).
  • Strong IT skills, including EMIS Web (or willingness to train).
  • Excellent communication and interpersonal skills.
  • Ability to prioritise workload and work under pressure.
  • High level of accuracy and attention to detail.
  • Commitment to confidentiality and patient-centred care.
  • Experience in clinical triage or signposting.
  • Experience with prescribing workflows.
  • Familiarity with NHS systems such as Docman, AccuRx, NHS App.
  • Previous experience in a care coordinator or admin lead role.
