A healthcare practice in Bolton seeks a part-time Care Co-ordinator to ensure efficient patient navigation and administrative support. The role involves handling enquiries, processing prescriptions, and managing electronic records. Candidates should have strong communication skills, relevant qualifications, and experience in a healthcare setting. Flexibility and excellent interpersonal skills are essential.
We are a large, friendly Practice with a team of over 50staff and a patient list of approximately14,500.
We have a multi-disciplinary clinical team which consists ofGP's, Advanced Practitioners and nursing team along with paramedics andpharmacists. This team is supported by a full administrative team comprisingsecretaries, administrators and care co-ordinators.
We are looking for a part-time Care Co-ordinator to workwithin our care navigation team at our two sites in Farnworth.
Monday: 8.00am 1.00pm
Tuesday 8.00am 6.30pm (30 minutes unpaid lunch break)
Wednesday: 1.30pm 6.30pm (30 minutes unpaid lunch break)
Total: 29 hours
Ensure an effective and efficient care navigation service isprovided to patients and any other visitors to the Practice.
Deal with general enquiries, make appointments and bookpatients in.
Ensure those patients with no prior appointment but who needurgent consultation are seen in a logical and non-disruptive manner.
Ensure that all new patients are aware of Practicearrangements and requirements and registration procedures are completedaccurately.
Process home visits in accordance with Practice policy,ensuring to accurately record all relevant details and referring to the dutyclinician accordingly.
Receive and make telephone calls as required, ensuringaccuracy of detail and prompt appropriate delivery of messages.
Enter patient information on to the computer as required,ensuring accuracy and adherence to data protection legislation.
Process prescriptions in accordance with Practice policy.
Ensure electronic records are up to date and accurate.
Advise patients of relevant charges for private services,ensuring pre-payment is made, receipts issued and paperwork completed forfinancial auditing.
Opening and locking up the premises when required,de-activating/activating the alarm and preparing the premises to receivepatients and ensure the building is totally secure.
