Job Summary

The role of the Care Co‑ordinator is pivotal within primary care as the interface between service users, carers, primary care, secondary care, community care, social care, mental health, out of hours and voluntary organisations. The Care Co‑ordinator will be part of the ARCH Integrated Community team and working alongside GP practices within Amber Valley. The Care Co‑ordinators provide an administrative, non‑clinical role, and applications are welcome from anyone with health or social care experience. We are looking for excellent communicators who have the enthusiasm and motivation to improve people's lives.

Main duties of the job

The Care Co‑ordinator is a pivotal role within the team and will be the interface between service users, carers, primary care, secondary care, community care, social care, mental health, out of hours and voluntary organisations.

To maintain IT‑based information systems and take responsibility for the production of relevant performance data.

The Care Co‑ordinator will contribute to tackling inequalities in health and social care particularly regarding individuals with long‑term conditions. An ethos of promotion of independence and partnership‑working is integral to this post.

Person Specification
Essential
  • NVQ Level 3 Business Administration or equivalent.
  • European Computer Driving Licence or equivalent.
  • Diploma/HNC level in Business Administration (or relevant experience).
  • Relevant experience of working with healthcare professionals/previous experience in the NHS or social care.
  • Able to demonstrate a clear understanding of working with confidential information and an understanding of service user confidentiality.
  • Experience in a multi‑disciplinary setting where influence and negotiation is required.
  • Experience in use of databases and administrative duties.
  • Knowledge/familiarity with medical terminology.
  • Working in a busy and demanding environment whilst delivering in a timely manner.
  • Willingness to undergo further training and development as the job develops.
  • Requires a flexible approach, and to be highly motivated. The role may need to be reviewed and developed in the future in line with changing priorities.
