A healthcare provider in Wales seeks a Cardiac Rehabilitation Specialist Nurse. An exciting opportunity has arisen within Cwm Taf Morgannwg for a Band 6 Specialist Nurse to join our Cardiac Rehabilitation team at Prince Charles Hospital. We are seeking a passionate, forward thinking individual with a background in Cardiology.
You will work as part of a team of specialist nurses, providing a high standard of nursing care to all patients accessing the Cardiac Rehabilitation service. You will be a key member of the multidisciplinary cardiac rehabilitation team, providing a comprehensive cardiac rehabilitation service for patients following a cardiac event. This will be in the form of education and exercise programmes to meet the individual needs of patients and may take place in a variety of settings;
To empower patients to self manage their condition, by developing individual care plans and promoting opportunities for long term self management.
