An exciting opportunity has arisen for a well-motivated, enthusiastic Cardiac Rehabilitation nurse to join the Hampshire and Isle of Wight Healthcare NHS Foundation Trust within the Isle of Wight's Heart Failure team.
This successful candidate will be responsible for providing specialist advice and review to patients with a confirmed chronic heart disease, as well as providing self-management education to support the prevention of unplanned admissions.
The team is responsible for delivering specialist care to patients in their own home as well in our community clinics and venues across the Isle of Wight.
