Cardiac Rehabilitation Nurse

An exciting opportunity has arisen for a well-motivated, enthusiastic Cardiac Rehabilitation nurse to join the Hampshire and Isle of Wight Healthcare NHS Foundation Trust within the Isle of Wight's Heart Failure team.

This successful candidate will be responsible for providing specialist advice and review to patients with a confirmed chronic heart disease, as well as providing self-management education to support the prevention of unplanned admissions.

The team is responsible for delivering specialist care to patients in their own home as well in our community clinics and venues across the Isle of Wight.

Main duties of the job

• To act as professional nursing resource to facilitate a multidisciplinary, multi-agency approach to the care and management of patients with CHD requiring Cardiac Rehabilitation.
• To be proactive in implementing recommendations from national and local guidelines.
• To develop and streamline the pathways of Cardiac Rehabilitation between primary and secondary care.
• To empower patients by promoting self-management and providing education aimed at preventing unplanned admissions.
• To participate in research and audits to meet NSF and NACR requirements.

Person Specification

Qualifications

• Registered Nurse. Registered with the nursing and midwifery council. Evidence of relevant degree level education. ENB 124/254 or equivalent cardiac nursing qualification.
• BACPR (British Association of Cardiac prevention and Rehabilitation) qualification. Independent prescribing.

Experience

• Experience of working within cardiology. Experience of implementing evidence-based practice. Knowledge of the adult learning and behaviour change. A practical and theoretical understanding of project management. Experience in independent working (change management).
• History taking and physical examination. Experience of developing clinical guidelines and policies. Experience in delivering teaching packages.

Additional Criteria

• This post requires highly developed physical skills particularly in the field of physical activity including an ability to partake in and teach physical exercise. There will be a requirement for the post holder to participate in moderate physical activity during the participation and delivery of exercise programmes. Knowledge of current issues in health and social care in relation to the NSF for Coronary Heart Disease including those relevant to Heart Failure. Understanding of current issues affecting clinical services including Clinical Governance and Clinical Audit. Awareness of the complex issues involved when working across organisations and disciplines. Understanding of professional and personal accountability in line with the standards laid out in the NMC code of professional conduct. Knowledge of equality of opportunity.
