Job description
Overview

The Colorectal Cancer Care Coordinator will play a vital role in supporting the delivery of high-quality, patient-centred cancer services. Working within the multidisciplinary colorectal team, the post holder will act as a key point of contact for patients, their families, and clinical staff throughout the colorectal cancer pathway. The role focuses on coordinating timely diagnostic investigations, treatments, and follow-up care, ensuring patients receive seamless support in line with national cancer targets and trust standards. The coordinator will manage patient databases and cancer tracking systems, maintaining accurate records to support pathway monitoring and reporting requirements. They will proactively identify and resolve potential delays, escalating issues when necessary, and assist with preparation of performance reports to drive service improvement. This role is subject to a closing date of 02 October 2025.

Salary and hours

37 hours 30 minutes per week.

Main duties of the job

The role requires excellent organisational and communication skills, proven ability to manage databases and information systems, and the capacity to prioritise a busy workload independently. Above all, the post holder will demonstrate a commitment to improving the experience of patients living with and beyond cancer, contributing to the delivery of safe, efficient, and compassionate care.

The post holder will coordinate the colorectal cancer patient pathway, ensuring timely progression from referral through diagnosis, treatment, and follow-up in line with national cancer waiting time targets. They will act as a central point of contact for patients, offering clear information, reassurance, and support while signposting to appropriate clinical or non-clinical services.

A key element of the role is managing and maintaining accurate patient records on cancer databases and tracking systems. This includes entering and validating data, monitoring progress, identifying potential delays, and escalating issues to the multidisciplinary team (MDT) as required. The coordinator will support the preparation of performance reports and contribute to audit and service improvement initiatives by providing accurate data and pathway information.

Job responsibilities

The post holder will work closely with consultants, clinical nurse specialists, and MDT members to ensure effective communication and smooth coordination of care. They will schedule and track diagnostic tests, outpatient appointments, and treatment dates, liaising with departments across the Trust to avoid unnecessary delays.

The role requires handling sensitive and confidential information, demonstrating empathy and professionalism in all patient interactions. The coordinator will be expected to manage competing priorities, work independently, and maintain excellent organisational skills to support delivery of a safe, efficient, and patient-focused service.

The post holder will also contribute to team meetings, pathway reviews, and the development of local processes to improve the overall quality and experience of colorectal cancer care.
