Job description

The Bristol Dementia Wellbeing Service is a joint venture between Alzheimers Society and Devon Partnership Trust. It provides Dementia services to people who are registered to a Bristol GP from assessment throughout the persons journey.

With co-production at the heart of what we do, we work holistically with the person and their carer and support network to support the people of Bristol with Dementia to live well.

An exciting opportunity has arisen within the service for Band 6 Senior Dementia Practitioner to join our innovative and award winning service on a part time basis (30 hours per week), although we welcome anyone seeking full fime hours to discuss with us before applying. Working as part of an MDT and in collaboration, the successful applicant will work closely with a designated group of Care Homes or GP surgeries using a liaison model of delivery. Working in the community using a range of core competencies (dementia focused) to work with the Care Home and/or GP to support the needs of people who present with a range of memory loss, behaviour changes and changes in cognitive functioning symptoms.

As part of the service, you will provide access to credible specialist dementia expertise for GPs, Primary Care and Care Home staff and others who are requiring support and expert advice on meeting the needs of those persons with a diagnosis of dementia.

It will include case management (design and co-ordination of care/support that is personalised and evaluated). The liaison with a wide range of statutory and non-statutory agencies is central to this role.

You will contribute to the clinical leadership of the team, providing support and informal supervision for other members and participating in team and service development and evaluation.

You will have access to supervision and appraisal to develop your own area of expertise to support the service to develop and meet the needs of the local community more effectively.

This role will be allocated to a designated hub but will be flexible to work across the city of Bristol as required.

The role will necessitate working some extended hours on our Access Point (Monday to Friday from 08.00-18.00). All clinical staff are provided with laptops to enable ease of flexible community working.

This is an opportunity for someone with a professional registration with transferable skills and experience working in Dementia, Mental Health, or Physical Health. This may be from a perspective of General Nursing, Mental Health or Learning Disability Nursing, Physiotherapy, Occupational Therapy, or Social Work.
