Responsibilities
- Undertake routine, urgent and same-day home visits to assess, diagnose, and manage patients with acute or chronic conditions.
- Provide an element of triage within the practice to assess clinical needs, prioritise requests, and allocate patients to appropriate services or clinicians.
- Deliver evidence-based care in line with current best practice and local/national guidelines.
- Prescribe and initiate treatment plans (if appropriately qualified and annotated on the professional register).
- Escalate and refer appropriately to GPs or senior clinicians where required.
- Maintain accurate, contemporaneous clinical records in accordance with NMC/HCPC/GMC and NHS standards.
- Contribute to reducing avoidable hospital admissions by supporting patients in the community.
- Work effectively with the wider multidisciplinary team, including GPs, nurses, pharmacists, social prescribers, and allied health professionals.
