Purpose of the Role
To provide flexible clinical support through home visiting and in-practice triage. The role ensures patients receive timely assessment, safe management, and appropriate onward referral, supporting the wider multidisciplinary team in delivering high-quality, patient-centered care.
Home Visiting undertake urgent and same-day visits to assess, diagnose, and manage acute or chronic conditions.
Clinical Triage provide face-to-face and telephone triage in practice, prioritising and signposting patients to the most appropriate clinician or service.
Autonomous Practice manage patients independently within scope of practice, escalating to GPs or senior clinicians when necessary.
Prescribing & Treatment prescribe and initiate care plans (if appropriately qualified).
Documentation maintain accurate, contemporaneous records using clinical systems (e.g. EMIS/SystmOne).
Collaborative Care work closely with GPs, nurses, pharmacists, and other team members to deliver safe and effective care.
Governance adhere to safeguarding, infection control, confidentiality, and professional standards.
Registered with HCPC (Paramedic) or NMC (ACP).
Experience in urgent care, community, or primary care settings.
Competence in triage and clinical prioritisation.
Ability to work independently and as part of a team.
Flexible, reliable, and able to travel to patient homes.
Desirable:
Independent Prescribing qualification (V300).
Experience in GP or PCN settings.
Knowledge of SystmOne.
