Passionate about transforming care for people living with frailty? An exciting opportunity has arisen within the Locality hubs for a Band 7 Clinical Practitioner for Planned Care to support the Locality Integrated Neighbourhood team.

We are seeking an experienced clinician with a strong background in the management of frailty and long-term conditions. The successful candidate will play a key role in leading high‑quality, person‑centred care, working collaboratively across multidisciplinary teams, and contributing to service development and improvement.

Job Title: Integrated Neighbourhood Long Term Condition Practitioner
Core Responsibilities
  • Provide advanced knowledge and clinical care for patients living with long‑term conditions (LTCs), including frailty.
  • Act as a generalist clinical expert, offering expertise across a range of long‑term conditions.
  • Support proactive, person‑centred care to improve outcomes and enhance quality of life for individuals with long‑term conditions.
  • Initiate and monitor treatment plans, ensuring effective management of health conditions.
  • Coordinate and deliver services through Integrated Neighbourhood Teams to help citizens stay healthy, manage their own conditions and remain well in their own homes.
  • Collaborate with GPs, hospital clinicians, community and voluntary sector services, and other health and social care professionals to promote independence, prevent deterioration and reduce unnecessary reliance on services.
  • Engage in service development and improvement activities, contributing to the strategic direction of the Integrated Neighbourhood practice.
Key Qualifications and Skills
  • Professional registration (NMC or HCPC) and a current, valid registration that allows for non‑medical prescribing.
  • Qualified non‑medical prescriber and completion of a recognised health assessment module.
  • Strong leadership and communication skills, capable of leading teams and influencing outcomes.
  • Commitment to person‑centred, evidence‑based practice and high standards of clinical care.
  • Experience in the management of frailty and long‑term conditions within community settings.
  • High levels of clinical judgement, discretion and decision‑making in complex care situations.
