  • Conduct medicines reconciliation on patient admission and discharge, ensuring accuracy and safety at every step.
  • Collaborate with multi-disciplinary teams to ensure coordinated patient care and effective communication across services.
  • Provide expert advice and education to patients, carers, and healthcare professionals regarding safe medication usage.
  • Maintain compliance with GPhC, ethical, and professional standards, ensuring excellence in pharmaceutical care.
  • Support medication audits and service development initiatives within your clinical specialty.
Qualifications
  • MPharm Degree or equivalent, with a minimum of one year's pre-registration training.
  • Current GPhC registration as a practicing Pharmacist (mandatory).
  • Postgraduate Clinical Diploma (or actively working towards it).
  • Proven hospital experience with confidence in clinical review and medication management.
