A healthcare organization in the United Kingdom is looking for a Primary Care Liaison Practitioner to assess and support individuals referred to their service. You will engage in initial assessments, manage referrals, and formulate care plans while working closely with primary care and mental health services. The ideal candidate possesses strong clinical judgement, effective communication skills, and a proactive approach to patient care. This role offers the opportunity to work both autonomously and collaboratively in a dynamic environment.
We are a strong and supportive team that is at the heart of connecting people with the right support at the right time.
We provide initial assessments and support conversations for people who may require secondary mental health services. We work closely with GPs, voluntary sector organizations, and both primary and secondary care, to ensure that people receive tailored, compassionate, and effective care. For example, we may meet someone who we then signpost on to support available within primary care. Or we may think that someone requires a period of support from secondary mental health services and refer them on.
We offer assessments and support conversations via telephone, video call and face to face and are based at Green Lane Hospital in Devizes. There will be a need for some regular office presence, but we also support home working. This role is perfect for those who thrive in a dynamic environment and want to be part of a team that values clinical expertise, holistic support, and person-centred care. If you're looking for a rewarding role where you can be autonomous and positively impact people’s lives, we’d love to hear from you.
If you have less experience in secondary mental health services or are newly qualified, we can also consider offering a fixed term Band 5 developmental role where you would be supported to develop into an autonomous Band 6 Specialist Practitioner.
As a Primary Care Liaison Practitioner, your key responsibility will be to complete initial assessments and support conversations for people referred to our service. This includes:
