We are looking for a Band 6 Locum Physiotherapist to join the Royal London Hospital team within the Barts Health NHS Foundation Trust. This full-time position offers an exciting opportunity for a senior physiotherapist with experience in acute hospital settings to coordinate and facilitate patient discharges. The successful candidate will play a key role in ensuring safe and efficient transfers from acute wards to onward services, contributing to the smooth operation of the hospital’s discharge processes. The role also involves contributing to the 7-day service, including covering public holidays.
Responsibilities:
As a Band 6 Physiotherapist, you will be responsible for facilitating discharge planning and coordinating the movement of patients from acute wards to onward services, ensuring continuity of care. This involves assessing patients, identifying discharge needs, and working closely with multidisciplinary teams, including nursing staff, occupational therapists, social workers, and medical teams. You will manage complex cases, ensuring that patients are safely discharged with appropriate support in place. The role requires flexibility to cover the 7-day service, including shifts over weekends and public holidays. You will be responsible for maintaining accurate records, liaising with external healthcare providers, and ensuring that patients receive the best possible care as they transition out of hospital.
