We are seeking a dedicated and experienced Band 6/7 Dietitian to support a meaningful project in Wolverhampton care homes. The focus of this initiative is to improve hydration among residents, with the aim of reducing the incidence of falls and urinary tract infections (UTIs). This is an excellent opportunity to make a tangible impact on patient outcomes while working collaboratively within a supportive healthcare environment.

Key Responsibilities

In this role, you will lead and contribute to a hydration-focused project across Wolverhampton care homes. You will assess the nutritional and hydration needs of residents, develop tailored care plans, and provide education and support to care home staff on best practices for hydration. You will also monitor and evaluate the effectiveness of interventions, collaborate with multidisciplinary teams to enhance resident wellbeing, and ensure accurate documentation in line with clinical guidelines.
