A leading healthcare provider in the UK seeks a dedicated team member to coordinate administrative functions for patients, ensuring smooth transitions from referral to discharge. Candidates must have GCSEs in English and Maths, excellent communication and organisational skills, and previous healthcare experience. The fixed-term position is based at Chase Farm Hospital, offering a salary of £32,199 to £34,876 per year.
Job Summary: Work as part of a team within a dedicated speciality/patient group to ensure that all administrative functions for patients from the time of GP referral to the time of discharge are coordinated in line with specific care pathway instructions to facilitate the smooth running of the patients' journey. Where a patient has multiple specialist needs, assist the patient in coordinating their pathways to reduce multiple appointments across the Trust.
Pay band: Band 4
Salary: £32,199 to £34,876 per year inclusive of HCAS
