Overview

We are looking for a dynamic and motivated Assistant Team Manager to join us within the Integrated Care and Support team within IUCT (Integrated Urgent Care team) at Crickets Lane Clinic. The team provides short term interventions to maximise independence and responds to urgent/crisis situations to enable people to continue living in their own home for as long as possible. We facilitate timely discharges from hospitals and the intermediate care facility. Our aim is to support people to live well at home by working in an integrated team which includes Social Workers/Assessors, Physiotherapists, Occupational Therapists, Nurses, Technical Care Instructors, Assistant Practitioners and Community Care Officers. We are committed to person centred and strength based practice, delivering high quality health and social care services, protecting the most vulnerable and creating strong, supportive, self-sufficient communities.

Responsibilities

As a key member of a busy, collaborative team you will be responsible for:

  • Managing and overseeing the day-to-day operations of the service
  • Line managing and providing support, advice and guidance to social care practitioners within the team
  • Assisting the Team Manager and Service Unit Manager in the organisation, management and supervision of Adult Social Care services
  • Contributing to strategic planning and management of the service and to the development of high-quality integrated services
  • Driving forward continuous improvements within the service using service user views, performance data and stakeholder feedback
  • Enhancing collaborative working across partners inside and outside the organisation to achieve better outcomes
About You / Qualifications

As a member of a supportive management team you will:

  • Lead the team ensuring the Unit functions are carried out effectively and in line with legislation, regulations, guidance, standards and local procedures
  • Hold a calm and reassuring approach in challenging situations and promote positive working practices
  • Lead with effective communication for both service users and staff teams
  • Participate in the organisation's transformation journey and ongoing professional development
  • Receive supervision, peer support and training to support continuous professional and personal development
  • Be able to engage in and contribute to service transformation to empower communities for adults and families
