Are you an experienced nurse looking to join a forward-thinking team? Do you want to support the local community in transformation by being part of the development of a service?

Our Great Yarmouth and Waveney Access and Assessment Team (AAT) are seeking a Band 6 Registered Mental Health Nurse to join their collaborative team.

AAT is the front door to all our adult mental health services in Great Yarmouth and Waveney. The team provides a community-based service to Great Yarmouth and Waveney market towns for adults over the age of 25. We screen, triage, and assess all referrals into secondary care via a blend of face to face, online and telephone appointments.

It is an exciting time to join as we're embarking on a new venture with our partners; working towards a truly integrated service model and being led by motivating and supportive leadership.

We're based at Oaktree, Northgate Hospital Great Yarmouth and work across the Waveney patch, you'll have a varied workload and will be supported by the wider MDT.

Main duties of the job

The role of the Assessment Nurse is to provide clinical interventions to our patients.

If appointed, you will:

Deliver clinical interventions / holistic mental health assessments.

. Co-produce safety plans

Offer support, supervision and informal training to your colleagues.

Contribute to multi-disciplinary team (MDT) meetings.

Undertake other clinical tasks within the team.

The ideal candidate will have some experience of working in the community supporting working age adults with mental health difficulties. You will need to have a sound understanding of the Care Programme Approach and be able to demonstrate your abilities in risk management. You will be part of the triage and assessment process managing referrals from our GP surgeries and other local services.

We enjoy excellent close working relationships with all statutory and non-statutory partners in developing Recovery based care plans. You can expect to have plenty of opportunities for joint working, career development and training with a clear appraisal structure.

Your role will be to:

  • Undertake the assessment, planning and delivery of direct patient care with minimal supervision, ensuring continuity of practice and working within a multi-disciplinary and multi-agency environment.
  • Communicate decisions about mental health care services, referrals and future support arrangements to people with a mental health care need.
  • Be responsible for accurate recording of all patients support, including assessments, reviews, day to day contact and discharges.

Qaulifications
  • Qualification in Mental Health Nursing,
  • Ongoing professional development working towards degree level ( if professional registration is following a diploma level course
  • Recognised Leadership/Management training
  • Able to demonstrate an appropriate level of knowledge of mental health practice from previous experience
  • Experience supervising others/students including preceptorship programme, student placements etc
  • Experience in supporting service development and quality improvement initiatives
