A health services organization is seeking an Assessment and Engagement Coordinator to support service delivery in HMP Durham. An exciting opportunity has arisen for a permanent post as Assessment and Engagement Coordinator to support Humankind’s service delivery within Reconnected to Health in HMP Durham.
You will work as part of a team to carry out assessments and deliver drug and alcohol recovery interventions. You will manage an agreed service user caseload, review ongoing care and treatment, and liaise closely with partners, family members and significant others to achieve and sustain recovery goals.
