A healthcare organization in Poole seeks a Specialist Mental Health Practitioner to work within the Shore Medical Primary Care Network. This full-time role focuses on providing integrated care and supporting local GP practices. Applicants should have a relevant mental health qualification, experience in a primary care setting, and strong skills in multi-disciplinary teamwork. Competitive salary offered ranging from £47,810 to £54,710 per year.
We have an exciting permanent opportunity for a Specialist Mental Health Practitioner to work in the Shore Medical Primary Care Network, comprising GP Practices based in east Dorset. The successful candidate will be an experienced Mental Health Practitioner with experience in a Primary Care and/or Community Mental Health Team setting. Applications from candidates requiring current Healthcare worker sponsorship to work in the UK are ineligible.
This role will work collaboratively between Primary Care and Secondary Mental Health Services as part of an Integrated Model of Care. The practitioner will focus services around local communities and GP practices to help rebuild and reconnect healthcare teams, providing targeted care according to population needs and supporting the PCN clinical strategy. The practitioner will facilitate onward access to secondary care-based services, third sector services and link with existing support services via the Primary Care Network.
Notes: This post is full-time (37.5 hours); consideration will be given to job sharing or part-time working. Employment requires an Enhanced DBS check (the Trust will cover the cost). Applicants who subscribe to the DBS update service and can present a valid DBS certificate may not require a new check.
