We are seeking to recruit an enthusiastic and motivated Advanced Practitioner (Nurses and Paramedics may apply) to join our friendly and supportive team. This position requires working independently, but with support from the GP team as needed. Experience in Minor ailments and Prescribing is expected.
The position offers a competitive salary with 6 weeks holiday plus Bank Holidays (Pro-rata) and an NHS Pension.
We are a rural practice based across 2 sites in the villages of Kirton Lindsey and Scotter and it will be necessary to work across both sites as required. We currently have approximately 10,700 patients, which is ever increasing. We have a CQC rating of GOOD. We use SystmOne (clinical system), SystmConnect and AccuRx (Online).
Main duties of the job
- Develop, implement and embed health promotion and wellbeing programmes
- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
- Implement and evaluate individual specialised treatment plans for chronic disease patients
- Identify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects to the patient health
- Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
- Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children
- Request and collect pathological specimens. Upon receipt, to process and interpret pathology and other test results as required

- Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
- Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)
- Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

Person Specification
Experience
- Minimum 5 years post‑registration experience.
- Prioritising needs and making informed decisions.
- Managing long term conditions.
- Use of primary care IT systems & working in primary care.
- Nurse led triage.
- Use of Systm1 - clinical system.
- Introducing and managing change.
Qualifications
- Relevant nursing degree masters degree or equivalent post‑registration education to meet the RCN recommendations for Advanced Practice.
- Qualified Nurse Prescriber.
