Job summary

A local medical practice in Greater London is looking for an Advanced Nurse Practitioner to join their team on a one-year fixed-term contract. The successful candidate will provide high quality clinical assessments and manage a variety of conditions, including chronic diseases. Job description

We have an exciting opportunity for an Advanced Nurse Practitioner to join our team at The Wick Surgery, on a one year fixed-term contract.

Main duties of the job

The postholder will provide high quality clinical assessment and treatment for a range of services including but not exclusive of chronic disease management and minor ailments clinics.

The aim of this role is to support the GP's to manage the growing demand by face to face and telephone consultations.

About us

The Surgery is based in Hackney and has a growing population of over 8000 patients.

We have a growing staff base consisting of GP's, Physicians Associates, Pharmacists, Practice Nurse, Sexual Health Nurse and Healthcare Assistants.

Job responsibilities

To deliver clinical nursing practice at an advanced level using expert clinical skills and to deliver appropriate and safe care in line with the NICE guidelines.

  • Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
  • Management of minor conditions
  • Management of acute and chronic conditions, including drug and non-drug-based treatment methods into a management plan.
  • Refers patients to other members of the multi-disciplinary practice teams, secondary care organisations and other organisations as necessary.
  • Communicates confidential and sensitive information to patients, relatives and carers in relation to their condition.
  • Provide highly specialised care advice beyond the scope of normal Nursing practice e.g. care treatment plans, making referrals as appropriate (including fast track referrals).
  • Contribute to local and national practice targets
  • To monitor and meet care outcomes against standards and recommend or initiate changes as necessary.
  • Management of health screening programmes and ensures other team members are aware of health priorities and screening objectives and programmes.
  • Makes referrals to appropriate agencies where necessary
  • Providing assessment, screening services, health education advice:
  • Produce accurate and complete records of patient consultation, consistent with legislation, policies and procedures
  • Participate in the administrative and professional responsibilities of the practice team
  • Accurate and legible notes of all consultations and treatments are recorded in the patients notes
  • Ensure clinical systems kept up to date, recording and/or amending accurate details
  • Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice
  • Attend and participate in practice meetings as required
  • Maintain up to date skills and knowledge, maintaining awareness of professional issues at an advanced level.
  • Assess effectiveness of care delivery through self-reflection and peer review, as well as benchmarking and formal evaluation.
  • Regularly participate in clinical supervision.
  • To support in the management of infection control in the practice and ensure all staff are aware of current procedures
