Job description
Overview

We are redesigning our services and looking for full and part time Advanced Clinical Practitioners to join our Community Hospital team as core members of an interdisciplinary service. Our Community Hospitals provide recovery and rehabilitation for people with Frailty. ACPs are clinical experts in Frailty and fulfil the 4 domains of Advanced Clinical Practice.

Responsibilities
  • Practice at an advanced clinical level with high levels of judgment, discretion and decision making in complex clinical situations.
  • Deliver advanced clinical practice within the field of frailty and contribute to clinical excellence.
  • Provide leadership and expert professional advice to patients, carers and colleagues.
  • Develop and provide education and training to other staff and trainees.
  • Contribute to service development, evaluation, research and audit.
  • Spend approximately 80% of the post holder’s portfolio in direct autonomous clinical practice.
  • Be involved in service redesign, quality improvement and research as part of the MDT, with the aim of developing a centre of excellence in care for older people.
Requirements and Qualifications
  • Forward-thinking, innovative individuals with an interest in modern dynamic inpatient Frailty services.
  • Ability to meet the criteria in the person specification and demonstrate relevant experience in applications.
  • Awareness of flexible working possibilities and the ability to discuss flexible arrangements with the employer.
