The Advanced Clinical Practitioner role is an autonomous practitioner supporting the extension of the Urgent Community Response in Great Yarmouth and Waveney. This is a role requiring an innovative and tenacious clinician, with a proven track record or recognised potential for embracing and leading change. The role will be a part of our Primary Care Home teams (integrated nursing and therapy). You will join the clinical leadership team alongside the Lead Nurse, Integrated Care Lead, Therapy Lead and Matron who work in partnership with Primary, Social and Acute care with the aim to build a healthier community in Great Yarmouth and Waveney by supporting people to be cared for at home for as long as possible.

Applications welcomed from clinicians new to Community working; support to transition to community practice will be provided.

Please note this post could be offered as a Band 7 development post for the right candidate who is able to study at level 7.

Main duties of the job

You will be required to consult, assess, and treat patients who are referred with undifferentiated, unpredictable, varied and complex needs, ensuring holistic assessment, prompt treatment and crisis care planning is undertaken to directly prevent unnecessary acute hospital admission. The advanced clinical level of this role will require you to be able to undertake advanced level comprehensive assessment including history taking, physical examination of the patient and a robust systems review.

You should be able to assess and manage frailty and complex chronic disease, identify deterioration and implement immediate strategies to reduce the risk of acute admission. You will need to be able to recognise the early symptoms of disease exacerbation, acute illness and injuries, based on an understanding of chronic diseases, the disease process, current evidence and practice standards.

The ACP will act as an autonomous, accountable practitioner working on their own initiative and in collaboration with nursing, medical and allied health professionals to assess individual patient needs, initiate investigations, determine a differential diagnosis and initiate appropriate holistic evidence based treatment and care. The ACP will be capable of making initial and follow up assessments of patients both in stable and unstable phases of their disease. They will use diagnostic skills including full history, appropriate clinical examination and required.

The Primary Care Home (PCH) is a multidisciplinary team working closely with GP's and other community partners to provide an excellent standard of person centered care to adults in their own homes, or residential care settings, supporting patients to self manage whenever possible. The service uses SystmOne as a clinical IT system to support mobile working.

Job responsibilities

Work autonomously as an advancedpractitioner within the specialty, managing a caseload of patients deliveringindividualised direct patient care.

Direct responsibility forassessment, examination, investigation, and diagnosis of patients within theirarea of work.

The ACP will beresponsible for the provision of effective, efficient and appropriate highquality care and act at all times in accordance within the relevant professionsCode of Conduct within their level of competence and agreed protocols.

Appropriately treat patients,resulting in the safe management and appropriate referral or discharge ofpatients with undifferentiated and undiagnosed presentations.

Receive referrals via a variety ofsources, including engagement with East of England Ambulance Service toidentify patients suitable for management within community setting. Manage owncaseload and clinical priorities according to agreed protocols and workingpractices.

Undertake a variety of clinicalskills and provide treatment / advice as per speciality and scope of practice.Using agreed protocols of clinical practice and professional guidelines. Thisservice may be provided within primary, community or home care settings.

Within scope of practice andclinical competence request and / or undertake diagnostic procedures andclinical investigations related to plans of care.

Utilise scope of practice toundertake Non-Medical Prescribing role and provide advice on medicinemanagement issues associated with the patient specialty group. Work within ECCHpolicy for Medicines Management.

Utilise advanced knowledge andskills relating to the speciality to provide specialist advice to other membersof the multidisciplinary team based on patient assessment.

Providea seamless, high quality service from referral through to assessment,diagnosis, treatment and review, referring to other specialists as required.Provide guidance to staff, patients and their families and carers on pathwaynavigation.

Leadin the development and updating of referral guidelines and policies for theservice.

Usehighly developed communication skills to effectively communicate withcolleagues, patients and their relatives / carers, making reports and liaisingas required with medical staff and other members of the multidisciplinary teamboth verbally and in writing.

Useprofessional judgement to act as an advocate for patients to ensure a patientfocused approach to the delivery of care. Support and enable patients andcarers to make informed decisions relating to their treatment and management.Escalate any concerns via the nursing / professional structures as required.

Worktowards safe and timely discharge and / or transfer of care of patients from orbetween hospitals and services and healthcare professionals, ensuring barriersto discharge / transfer are identified and acted upon appropriately.

Reportand raise concerns related to Safeguarding, accessing advice and support asrequired. Be conversant with ECCH policies for safeguarding including theassessment of mental capacity, and consideration of Deprivation of LibertySafeguards (DOLS) and the application of the principles of Prevent (counterterrorism awareness).

Undertaketraining to develop further advanced clinical practice roles required by theservice to provide a high standard of patient care.

Providehighly visible and accessible professional leadership and demonstrate expertknowledge and standards of clinical practice.

Promoteteam working, build rapport and collaborative working practices withmultidisciplinary team. Liaise with inter-hospital departments and personnelacross organisational and professional groups. Ensuring effective communicationand interpersonal skills with other disciplines and organisations.

Leadand support the development of the role according to changing patients needs,service requirements and evidence base practice.

Utilisethe highest level of interpersonal and communication skills when dealing withcomplex, sensitive or emergency situations, utilise persuasion, motivation andcoaching to facilitate best patient outcomes, supporting patients to overcomebarriers to communication where necessary.

Leadand actively participate in service/ departmental projects, quality initiativesand statutory accreditation processes. This will include the setting andmonitoring of clinical standards of care.
