Overview

Advanced Clinical Pharmacist - Bank. You will join on a bank basis to develop and deliver the medicines optimisation and safety agenda alongside a team of clinical pharmacists. The role requires expertise in medication management and patient care.

What you will be doing

• Processing repeat and non-repeat medication requests
• Clinical medication reviews with patients to optimise their treatment
• Support the development and implementation of prescribing policies
• Reconcile medicines after hospital discharge and ensure patients receive required treatments
• Monitor and review prescribing patterns to ensure compliance with the Pan Mersey Formulary
• Answer medicine-related queries from healthcare staff and patients
• Contribute to audits and quality improvement initiatives
• Provide clinical advice
• Support public health campaigns, including immunisations and flu vaccination initiatives
• Ensure compliance with CQC standards and health, safety, and infection control policies
• Work collaboratively with the wider healthcare team, including GPs, nurses, and other professionals, to deliver exceptional patient care
• Advocate for patients, ensuring clear communication about their treatment options
• Contribute to fostering a supportive and efficient team environment

What you will need

• A degree in pharmacy
• Be an independent prescriber
• A minimum of 2 years post-qualification experience
• A minimum of 2 years experience of working in general practice
• Excellent written and verbal communication skills
• Ability to work with minimal supervision remotely, with excellent organisation skills

Salary: £30.00 per hour
