Are you ready for an exciting opportunity in Portsmouth? We are looking for an Adult Social Worker to join the Hospital Discharge Team for a locum position at £36 hourly. This full-time role is based mainly on “The Limes” ward within St James Hospital – Milton. Over a period of 3 months, you’ll manage a case load of patients, ensuring their smooth discharge, either back to community options or by sourcing suitable placements.
