Job description

This is a new and exciting collaboration between Dementia UK and 6 Primary Care Networks in Sheffield . Admiral Nurses are specialist dementia nurses who offer expert practical, clinical and emotional support to families involved in the care of a person living with dementia with complex needs. Based within multi-disciplinary/agency community teams and working alongside nurses, therapists, primary care, social care and third sector providers, amongst others, you will also benefit from being a part of a Clinical Network of Admiral Nurses .

It will be of especial interest to nurses looking to enhancetheir leadership and communication skills. We are looking for a skilledregistered nurse, either already working as an Admiral Nurse, or from anadult/mental health or learning disability background. You will have experience of providing personcentred care and possess experience of delivering bio-psychosocialinterventions to improve the experience and quality of life of PLD and theirfamilies. You will also be required toestablish and maintain close and collaborative working arrangements with thelocal authority colleagues, voluntary organisations and charitable groups whoalso form part of the Dementia in Primary Care Project.

Dementia UK will support you to maintain and strengthen your clinical and leadership expertise as a specialist Admiral Nurse through regular clinical supervision and practice and professional development opportunities.

Main duties of the job

The Admiral Nurse will support, deliver andcoordinate relationship-centred dementia care within Dovercourt Surgery and the 6 PCNsinvolved in the project, in order toimprove the experience of carers/families affected by dementia withcomplex needs.

Inline with the Admiral Nurse Competency framework at a specialist level, thepost holder will provide:

Specialist bio-psychosocial assessment andsupport for people with dementia and families with complex needs

Facilitation of education and training for staffin the delivery of evidence-based dementia car

Clinical leadership with policy development and qualityimprovements in relation to dementia care

Consultancy to other health and social careprofessionals regarding dementia

Leadershipand management for one Band 6 Admiral Nurse involved in the project

Dovercourt Surgery is the host practice. This post will be delivering care across 6 PCNs in Sheffield.

Job responsibilities

Clinical

Provideclinical leadership on the care of people with dementia and theircarers/families with complex care needs from peri diagnosis to post bereavementsupport

Providespecialist nursing assessment, using the Admiral Nurse Assessment Framework forcarers/families affected by dementia

Providea range of bio-psychosocial interventions to promote health and wellbeing forpeople with dementia and their carers/families

Workcollaboratively with other professionals to improve the identification andmanagement of co morbidities and frailty for people with dementia and theircarers/families

Developand support care plans for people with dementia and families including positiverisk management

Supportpeople with dementia, carers/families and staff to understand and respond tochanges in behaviour and relationships as a result of dementia.

Providecare coordination for a clinical caseload of families with complex needs asidentified within triage assessment in clinics or via home visit if required

Ensurethe safeguarding of vulnerable adults and that all legal requirements relatingto Mental Capacity Act 2005, Mental Health Act 1983 (amended 2007), andDeprivation of Libert Safeguards (2009)[1] are considered and adheredto in relation to patient care

Actas an advocate for people with dementia and their carers/families to promotetheir rights and best interests

Participateand lead on case conferences, continuing health care processes and best interestmeetings where required

Workcollaboratively with health, social and voluntary sector services to achievecoordinated and integrated care

Workin partnership with colleagues to promote the use of advance care planning forpeople with dementia and to support and inform families, around advance careplanning, best interest decisions and end of life care

Manageand advise on complex care transitions for people with dementia and theircarers/families to reduce the frequency of crisis situations

Ensurethat care is delivered in accordance with relevant local and nationalguidelines and all relevant Dovercourt Surgery (the host practice for theproject) policies.

Recognisethe limits of own competency and professional boundaries and make appropriateand timely referrals to other services where required

Education and Leadership

Facilitate and support the development and delivery ofinnovative dementia training and education to staff, to improve competence and confidence, inline with best practice and guidance

Participatein and/or lead formal training events promoting best practice in dementia careand disseminating Admiral Nursing work

Provideprofessional and clinicalleadership and mentorship, acting as a clinical role model on the delivery ofevidence-based practice in dementia care within Dovercourt Surgery.

Adviseand support on the development and delivery of work streams as part of DovercourtSurgerys dementia strategy, where appropriate

Workproactively with key local and national stakeholders to develop more integratedcare pathways and holistic models of care, through dissemination of specialistskills and knowledge

Contributeto the delivery of local policy, procedures and protocol for DovercourtSurgery and the 6 PCNs involved in the project, to improve identification,assessment, ongoing support for people with dementia and their family carers

Evaluation and Audit

Supportthe audit and evaluation of quality improvements and initiatives aimed atimproving the delivery of dementia care within Dovercourt Surgery, whereappropriate to role

Collectand review quantitative and qualitative data, as agreed, to evaluate and auditthe Admiral Nurse Service and make necessary improvements

Undertakeanalysis and evaluation and make any recommendations to the PCNs DementiaStrategy Group, the Citywide Dementia Strategy and the Primary Care DementiaProject work.

Ensureservice information and data recorded is relevant, accurate, complete andcaptured in a contemporaneous manner

Supportevaluation of workforce development in dementia to improve staff knowledge andskills across the host organisation

Professional

Ensure all nursing practice is carried out inaccordance with the agreed policies and procedures of Dovercourt Surgery(the host employer for the project) and the NMC Code

Attend and actively participate in practicedevelopment, clinical supervision, and continuing professional developmentsupported by Dementia UK and underpinned by the Admiral Nurse Competencyframework

Developown competence through use of the Admiral Nurse Competency Framework andportfolio development using resources provided by Dementia UK

Tomaintain professional registration in line with NMC guidance, includingre-validation

Contribute to and lead the developmentof policies, procedures and protocols relevant to dementia and Admiral Nursingwithin Dovercourt Surgery and the 6 PCNs involved in the project includingthe Standard Operational Policy

Ensurecompliance with all clinical governance requirements of the host employer andthe 6 PCNs involved in the project.

Participatein clinical supervision/mentoring within the 6 PCNs where appropriate.

Maintainup-to-date knowledge of evidence-based recommendations on supporting peoplewith dementia and their carers/families

Maintainpositive working relationships with Dementia UK

Managerial

Undertake staff appraisal/managerialsupervision of Band 6 Admiral Nurse for the duration of the fixed term.

Ensurethe recording and investigation of all accidents, complaints, untowardincidents and losses in accordance with Dovercourt Surgerys policy as hostemployer and for the 6 PCNs.

Managestaff sickness and absence according to the policies set out locally

Participatein recruitment of other healthcare professionals where appropriate

Leadon Admiral Nurse steering groups with Dementia UK, Dovercourt Surgery as thehost employer and the 6 PCNs.
