PURPOSE AND SCOPE:

The professional registered nurse Outpatient RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is responsible for providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for assigned patients. It may include delegating tasks to direct care staff such as RNs, LVN/LPNs, and Patient Care Technicians. As part of the End Stage Kidney Disease (ESKD) team, this role involves decision-making, teaching, leadership, and quality improvement activities to enhance patient outcomes and facility operations.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

• Perform duties under supervision and guidance from educators, preceptors, or collaborating RNs.
• Collect and analyze patient data during hemodialysis, document findings, and adjust treatment plans accordingly.
• Educate patients and families about care, transplant, and treatment options.
• Ensure safe and effective delivery of hemodialysis, collaborating with care staff.
• Administer medications as prescribed, document, and evaluate effectiveness.
• Assist with emergency responses when needed.
• Participate in staff training, orientation, and serve as a resource for the team.
• Manage laboratory procedures and follow up on missed collections.
• Update nursing assessments and care plans collaboratively.
• Complete CAP requirements for career advancement.
• Perform additional duties as assigned.

PHYSICAL DEMANDS AND WORKING CONDITIONS:

• Involves heavy lifting, moving patients, assisting with ambulation, and prolonged standing.
• May require moving equipment up to 200 lbs. and lifting solutions of up to 30 lbs.
• Work environment is a healthcare setting with temperature control and moderate noise.
• Potential exposure to infectious materials.
• Includes desk and computer work, patient and staff interaction, and possible travel.

SUPERVISION:

Oversee Patient Care Technicians, LPNs, LVNs, or RNs as a Team Leader or Nurse in Charge after meeting experience and training requirements.

EDUCATION and LICENSURE:

• Graduate of an accredited Nursing school.
• Valid state nursing license.
• CPR/BLS certification.

EXPERIENCE AND SKILLS:

• Entry-level RN with less than 1 year of nephrology nursing experience in the last two years.
• Minimum 9 months RN experience preferred.
• Chronic/acute hemodialysis experience preferred.
