Job summary

An Indigenous community organization in Ontario is seeking a Transitional Housing Case Manager. The Case Manager will provide support for individuals transitioning from emergency shelters to transitional housing, ensure a safe environment, and engage with community services. The ideal candidate should have a background in social work or a related field, possess effective communication skills, and understand Indigenous culture and systemic barriers. The role requires a valid driver’s license and may entail flexible hours.

Qualifications

• Experience in social service delivery, particularly frontline work.
• Valid First Aid & CPR certification is required.
• Training in Non-Violent Crisis Intervention and Trauma Informed Care is an asset.

Responsibilities

• Conduct intakes, assessments, and create personal case plans.
• Collaborate with community service partners for case management.
• Coordinate outreach and psychoeducational activities.
• Maintain records ensuring client privacy and confidentiality.
• Ensure compliance with policies and procedures.
