Reporting to the Senior Manager, the Telehomecare Clinician provides remote patient monitoring, health coaching, and disease-specific education to patients. The Telehomecare Clinician will remotely monitor and coach patients with complex chronic diseases. This role is based on a collaborative model of care that focuses on enabling the patient to better manage his/her chronic disease through the support of clinician monitoring and coaching. The Telehomecare Clinician will work in collaboration with the patients’ circle of care.

Please note that the Telehomecare Clinician will be responsible for both remote care and remote surgical monitoring.

Responsibilities
  • Conduct eligibility for any referred patients
  • Obtain informed patient consent to participate
  • Create and implement a customized care plan with patients that best meet the patient needs
  • Assist patients in establishing and implementing self-management goals
  • Support patients in medication awareness and management
  • Monitor patients and respond to alerts using established clinical protocols and guidelines
  • Respond to patient phone/email inquiries and determine clinical next steps
  • Deliver patient education using validated educational tools and resources
  • Conduct regular and just-in-time health coaching sessions with patients
  • Remotely monitor and manage an assigned patient case load
  • Actively participate in ongoing training/education sessions and Community of Practice offered through the Telehomecare Centre
  • Participate as a member of the Telehomecare Team to provide feedback into continuous quality improvement initiatives
  • Participate in program evaluation activities when required
  • Conduct home visits and face to face assessments where required.
  • Participate as a member of a selected number of committees representing Telehomecare issues

Qualifications
  • University degree preferred in Nursing, and /or equivalent
  • Holds current appropriate unrestricted registration as a member of the College of Nurses of Ontario
  • Experience in patient centered models of care where patient experience is a priority
  • Excellent knowledge and expertise working with complex chronic diseases including, but not limited to: CHF, COPD, palliative care, mental health, chronic kidney disease and diabetes
  • Foundational knowledge in clinical practice, decision-making, critical thinking, leadership, research utilization, resource management and problem solving skills
  • Proficient with adult health education delivery, health coaching techniques and patient self-management support
  • Protect patient confidentiality in accordance with legislation and college requirements
Preferred qualifications
  • Completion of Respiratory Therapy program
  • Other relevant certifications (i.e. CDE, CRE, CNA designations, TEACH, Choices & Changes, etc.)
  • ICU, Cardiology Unit, or ER experience
  • Experience with motivational interviewing counseling
  • Ability to speak French or another second language
Hours of work

All hours of operation (Initially Monday-Friday 0830-1630; 70 hours per bi-weekly pay period) subject to change as per the Collective Agreement.

Position location and travel

Hybrid work model, subject to change.

Regular travel within the geographical region of Ontario Health atHome Central East may be required. A valid Ontario driver’s license and access to a vehicle are necessary.
